Prescriptions

You can order a prescription in the following ways:- 

    - in person at either surgery

   - by post, enclosing a stamped addressed envelope

   - by fax   Congresbury= 01934 834165   Yatton 01934= 01934 876085

   - by taking your prescription counterfoil, with the items you require ticked, to 

      Lloyds Pharmacy Yatton,  Day Lewis Congresbury, GP Care at Yatton Surgery 

      or Clevedon Tesco.  The pharmacy will then bring your request up to the surgery

      for the prescription to be issued in the normal way.

   - email Day Lewis Pharmacy, Congresbury  congresbury@daylewisplc.com

   - online by completing  the form below, allowing 48 hours before collection from the Surgery

   - ringing Lloyds Pharmacy Yatton 01934 832236 (nb not during lunchhour 1pm to 2pm)

   - ringing GP Care at Yatton Surgery during their opening hours 01934 832366 

Requests received by 12pm will be ready for collection at the surgery by 12.00pm 2 days later.  On Fridays, if received by 12pm, prescriptions will be ready for collection after 12pm on Tuesdays. If you would like your prescription to be sent to a chemist, please indicate this on the form below and allow at least 3 working days before collection. If you are unable to collect your prescription and require a delivery, you must arrange this with the pharmacy directly. RE POWER FAILURE PROBLEMS OVER THE  WEEKEND 21ST/22ND AUGUST.  WE ARE NOW ABLE TO ACCEPT YOUR ONLINE REQUEST .  HOWEVER PLEASE RESEND ANY REQUESTS SUBMITTED BETWEEN SATURDAY 21ST UNTIL LUNCHTIME WEDNESDAY 25TH AUGUST AS THESE HAVE NOT COME THROUGH.  WE APOLOGISE FOR ANY INCONVENIENCE THIS MAY HAVE CAUSED.
